These are home care instructions to treat painful muscle spasms and manage your clicking and popping joints. This is part of your responsibility in eliminating your TMJ problem. The therapy is to be done twice each day. In the morning before breakfast and in the evening before retiring.

This is the same procedure you performed at our office when it took you only minutes to cycle the painful spasms. Each time you treat yourself it will take less time to eliminate the pain.

  1. Place the rubberized deprogrammer between your front teeth, slide your jaw forward, then all the way back.
  2. Immediately apply the hot wet, compresses or the towel over both your joints. The compresses must be kept hot throughout your treatment.
  3. Start biting on the deprogrammer for 6 seconds, then relax your biting for 5-6 seconds. Bite again for 6 seconds, etc. until the pain is eliminated.
  4. After the pain is eliminated you should hold the deprogrammer between your teeth with only light pressure, continue to apply the hot compresses for another 5 minutes.
  5. Upon removal of the deprogrammer do not alarmed that your teeth do not fit together correctly. This is to be expected. Do not be alarmed if it hurts to open wide immediately after your treatment.

Do not be alarmed if the character of timing of your click or pop is different for it usually will be.

The gauze placed by your dentist should remain in place and under pressure for a minimum of 45 minutes (then disposed of thoughtfully). After that, if necessary, or if you feel that it is still bleeding a lot, you may bite on another piece of folded gauze (supplied in pack) for another 30 minutes.

For the rest of the 1st day, DO NOT keep spiting or rinsing your mouth. That will only make the bleeding worse. It is safe to swallow your own blood, and usually it is just a very small amount of blood mixing with your own saliva. Take two pain killers if feel needed (Panadol or Neurofen supplied in pack). Slight pain is to be expected and can be treated with pain killers. If pain is intense, may take Panadeine Forte (supplied in pack as well).

DO NOT eat anything, or drink anything hot, until the anaesthetic wears off and the numbness goes away. You may then eat anything you feel comfortable, however hot food/drink on the day of extraction will usually cause slight discomfort.

Brushing your teeth is still recommended on the same day. Just be gentle. Start warm salt water baths (1 heaped teaspoon of salt water in a glass of luke warm water) in the extraction area on the same night of the extraction. This will keep the socket clean and help prevent secondary infection. Avoid rinsing too vigorously as this may induce bleeding. Mouth baths should be carried out after every meal for at least 5 days.

DO NOT smoke or drink alcohol for the next few days to avoid delaying healing. No vigorous exercise for the first 24 hours. If you develop a Dry Socket (bad taste, severe pain) a few days after the extraction, contact us immediately for an emergency appointment so that we can place a special medicament dressing for you. There is no extra charge for this service. Antibiotic from your medical GP will only temporarily suppress it.

  1. Thoroughly brush and floss your teeth. 
  2. In an anti-clockwise motion, twist and pull off the clear plastic cap from the end of the syringe.
  3. Place the mixing nozzle on the end of the syringe and secure by twisting the mixing nozzle in a clockwise motion.
  4. Place a small amount of gel (tear drop, equivalent to about 1/3 of size of the tiny blue rubber cap) in each tooth compartment of the tray.
  5. Place tray in your mouth for at least 45min, and maximum of one hour only. Any excess gel that squeezes out onto the gums should be wiped away with a tissue immediately. Overloading trays with gel may cause tooth sensitivity or gum irritation. A little gel goes a long way! This should be done for about 14 days consecutively, not more than 21 days.
  6. If you experience excessive sensitivity, you may use the gel on alternate days instead. It will still work just fine. You just need to do it over a longer period to achieve the same result. You may also rub Sensodyne/ Oral B Sensitivity/ Colgate Sensitivity toothpaste in the area (several times per day) after bleaching to dampen the sensitivity.
  7. While wearing your trays you may notice “bubbling” within your trays. This bubbling is part of the bleaching process. You may also notice opaque white patches or ‘crack lines’ on your teeth. These are naturally there. Do not worry about those as they will disappear after the completion of the bleaching treatment.
  8. On removal of the tray gently wipe off the excess gel with a tissue and rinse your mouth thoroughly with water.
  9. Do not swallow. Do not brush your teeth immediately after the bleaching process.
  10. Rinse the tray with cold water, or gently clean with your toothbrush, then put them back into the case provided. Do not use hot water.
  11. Do not smoke, or drink any red wine, tea or coffee, or anything that may stain a white shirt badly (eg curry), within 2 hours after each bleaching application.
  12. Store bleaching tubes and trays in a dry, cool place.

Due to my interest in Paediatric Dentistry, I got gradually drawn into Orthodontics and Occlusion, then leading on to TMJ Dysfunction/pain and Obstructive Sleep Apnoea. I have attended many courses, workshops, conferences and seminars over the years, and Bob Walker's name first came to me quite many years ago.

Well, it works for ~70% of the population. You won't necessarily go laughing gaga, but it does seem to really help most people get more relaxed.
